What To Bring?
For your first visit, please be sure to arrive 15 minutes before your appointment time and bring the following information with you:
● Your completed Dental History and Patient Information forms which you can download using the buttons below.
● Your Dental insurance card (if you have Dental insurance).
● A list of current medications you are taking including dosage.
Guidelines
This is a membership plan, not a Dental Insurance Plan. It cannot be used:
- In conjunction with another dental plan or dental insurance plan
- For referrals to specialists
- For costs of dental care which are covered under automobile or medical insurance
- For services of injuries covered under workers' compensation
- Plan must be paid in full at the time of enrollment
- Plan will run on a benefit year determined by the first date of service
- Premiums are non-refundable even if the plan is unused by the participant in the given benefit year
- Patient’s portion of the bill is due at time of service

Insurance Plans Accepted
ViVa Dental Studio accepts some PPO dental insurance plans, and we are happy to help you maximize your insurance benefits. Simply give us a call, and our front desk staff will check your insurance plan to make sure we accept it and also give you information about your coverage and benefits.
